Job Summary

We are seeking a compassionate and skilled Hybrid Nurse Case Manager to join our team. In this role, you will assess the needs of our recipients for case management intervention, utilizing comprehensive clinical information, medical history, and environmental factors. You will collaborate closely with healthcare providers to implement and monitor personalized care plans, ensuring optimal outcomes for our recipients. Additionally, you will educate recipients and their families about available community resources to support their ongoing care needs.

Key Responsibilities

Assess the need for case management intervention based on thorough evaluation of clinical information, medical history, and environmental factors.

Document assessments, develop personalized care plans, and establish timelines for intervention implementation.

Optimize available benefits and compare costs to facilitate the most effective care plan.

Educate recipients, families, and healthcare providers about community resources and services.

Collaborate with primary care physicians, specialists, and other providers to support treatment plans and achieve recipient-centered goals.

Continuously evaluate the effectiveness and efficiency of care plans, making adjustments as necessary.

Ensure appropriate referrals to healthcare providers, promoting cost-effective delivery of care.

Job Highlights

Schedule: Monday to Friday, 8:00 AM to 4:30 PM (no nights or weekends); travel time included within work hours.

Benefits: Full-time employees receive health benefits, paid time off (PTO), and eligibility for 401(k) with company match after 90 days of employment.

Compensation: $39.92 per hour.

Requirements

Active VA Registered Nurse license.

Minimum 3 years of clinical experience (medical surgical, behavioral health, etc.).

Experience in case management preferred; home health/home care experience a plus.

Proficient in computer applications and technology.
