The Full-Time Telehealth Nurse will remotely monitor patients, track and trend vital signs, virtually assess patients and work with the clinical care team to coordinate care for patients. The Telehealth Nurse will use excellent clinical and interpersonal skills to work with patients and the members of the clinical care team to provide high standards of clinical care. They will think critically and creatively about what is needed to enhance the patient’s care.

Role & Responsibilities:

  • Triage patients using protocols to determine what measures can be taken by the nurse and when to escalate to appropriate level of care.
  • Provide nursing assessment.
  • Monitor patient vital signs and survey responses, and triage, review, respond and document.
  • Review trends in patient data and changes in condition and communicate with members of the clinical care team.
  • Document interactions and interventions.
  • Educate patient on their disease process.
  • Educate patient and family in preventative care which promotes health and well-being.
  • Answer, direct, and follow up on incoming calls.
  • Contact patient as required for clinical follow-up.
  • Recognize urgent or emergent situations, seek assistance of the medical provider, and initiate appropriate emergency interventions as directed.

Qualifications:

  • Bachelor’s degree in nursing, preferred.
  • Emergency or critical care nursing experience preferred.
  • Current license to practice as a Registered Professional Nurse in all states
  • Proficiency with MAC, email, and web-based skills preferred.

Requirements:

  • Must be able to work from home with a HIPPA compliant environment.
  • Must be flexible on scheduling and available to work some holidays and weekends.

Base hourly rate for this position is $35 – $45/hr depending upon experience.
