Responsibilities:

The Travel Nurse participates in admissions, assessment, evaluations, and documentation to ensure that patients are receiving exceptional care. As a Travel Nurse you will:
  • Under the physician’s order, admit patients eligible for hospice services
  • Assess and evaluates patient needs/problems as outlined in the patient IDG Plan of Care; identify mutually agreed upon goals with patients and families
  • Document daily changes and updates to the patient care plans, including discharge planning, and notify attending and hospice physician through scheduled IDG meetings
  • Participate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection relating to patient care problems
  • Attend skilled nurse in-services and meetings, and interdisciplinary team meetings
  • Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications:

  • Current unencumbered Registered Nurse (RN) state license or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Three years of experience as a registered nurse in a licensed hospice or home health setting including team supervision
  • Current driver’s license and automobile in good working condition with proof of auto insurance
  • Demonstrates above average computer and EHR competency
  • Able to handle multiple and shifting assignments
  • Ability to make sound professional clinical judgment in a field setting

Posted Max Pay Rate: USD $104,468.00/Yr. Posted Min Pay Rate: USD $85,651.00/Yr. Pay Transparency: Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
