Central Coast Home Health is a well-established, locally owned organization known for its exceptional services across San Luis Obispo and Santa Barbara counties. As we continue to expand, we are looking for a dedicated part-time or full-time Palliative Care Case Manager to join our team in the Nipomo, Santa Maria, and Lompoc regions.

**Role Overview**

As a Palliative Care Nurse at Central Coast Home Health, you will be responsible for providing care to patients with life-limiting illnesses who are pursuing curative treatments. Your focus will be on managing symptoms, discussing advance directives and end-of-life decisions, and educating patients on their disease processes. Working closely with a diverse team that includes social workers, counselors, and therapists, you will have the opportunity to make a difference in the lives of your patients and their families.

**Key Responsibilities**

  • Provide in-home care to patients, allowing for more personalized attention and support
  • Collaborate with a multidisciplinary team to address the holistic needs of patients
  • Support patients in managing symptoms and navigating their healthcare decisions
  • Educate patients and families on disease processes and available resources

**Work Schedule**

Your regular work schedule will be Monday through Friday, 8am-5pm, with the option to choose between working 4 or 5 days a week. Additionally, there will be a weekend rotation approximately every 4 weeks to ensure comprehensive patient care coverage.
