Specialized monitoring, clinical evaluation and treatment of sick individuals through home visits/in the field, in close collaboration with home care. An overview of the team's treatment and client services. Participation in regular assessment of the status and performance evaluation of the team's specialized home care. Reception, processing and follow-up of requests and inquiries to the team. Counseling and communication with clients and relatives in cooperation with home nursing staff. Supervision of family meetings in cooperation with the team doctor and home care team leader according to procedure.
