We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Mobile Care Coordinator. In this role, you will work closely with patients, their families, and healthcare providers to ensure safe and effective care coordination and management. You will be crucial in guiding patients through complex healthcare situations, providing support, education, and advocacy.
Minimum of 10 years nursing experience Must be bilingual: English/Spanish Education: Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Nursing or related field preferred. Licensure: Active Registered Nurse (RN) license in the state of New Jersey. Experience: Minimum of 10 years of clinical nursing experience. (case management, chronic disease management, or patient advocacy experience a plus). Skills: Strong communication, listening, critical thinking, and organizational skills. Ability to work independently and as part of a multidisciplinary team. Flexible and solution-driven. Confident and empowered to advocate on behalf of patients at all levels of healthcare. Technology: Proficiency in electronic health records (EHR), and Microsoft Office Suite. Travel: This mobile role requires daily travel in a company-provided cobranded vehicle for in-person patient support.
Patient Advocacy: Serve as a dedicated advocate for patients, ensuring their needs and preferences are respected and prioritized throughout their healthcare journey. Care Coordination: Collaborate with healthcare providers, insurance companies, and community resources to coordinate comprehensive care plans tailored to individual patient needs. Case Management: Assess, plan, implement, and evaluate care strategies for patients with chronic conditions, post-acute care needs, and other complex health situations. In-Person Support: Provide on-site support to patients during critical healthcare events, such as hospitalizations, doctor, specialist appointments, and home visits. Education and Guidance: Educate patients and their families about their conditions, treatment options, and available healthcare resources. Documentation: Maintain accurate and up-to-date patient records, documenting all interactions and interventions thoroughly. Collaboration: Work closely with other team members, including social workers, clients, colleagues, and medical professionals, to ensure a holistic approach to patient care.
