Registered Nurse, RN/Case Manager - Home Care (Company Car and Gas Card provided)

Novant Health is hiring a Home Health Case Manager registered nurse for the standard shift for our New Hanover Regional Medical Center location in Wilmington, NC. Company car and gas card provided. We ’re seeking a registered nurse to join our team in our home health department. We are a very supportive team and always available to help each other out. In this position, you will be responsible for coordinating home care services which includes establishing and maintaining the patient’s plan of care. This includes the quality, customer service and financial management of your assigned caseload. Schedule: PRN, 0.1 FTE, 1-31 Hours per 2 weeks, standard hours shift, 8:00am - 5:00pm Department: Home Care – Skilled Nursing Unit Responsibilites: Conducts admissions, recerts, ROC, and discharges for the assigned case load. Conducts routine nursing visits as needed. Assesses patient needs. Coordinates patient care between the patient, caregiver, physician, and other appropriate disciplines. Responsible for the completion of all home health documentation.
