Education & Licensure: Registered Nurse (RN) with 3+ years of direct clinical care experience OR Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) with 2+ years of clinical practice. Active, valid, unrestricted license in the state of operations (or reciprocity). Compact license holders must obtain an active state license within 90 days of hire. Experience & Skills: 3+ years in wellness or managed care, addressing clinical issues with members/physicians. Knowledge of the health and wellness marketplace and employer trends. Strong analytical skills with experience in medical data analysis. Exceptional verbal and written communication skills for engaging with members, staff, and external resources. Proficiency in Microsoft Word, Excel, PowerPoint, databases, and web-based applications. Additional Requirements: Current driver's license, reliable transportation, and applicable insurance. Ability and willingness to travel within the assigned territory. Preferred Qualifications: 3+ years of clinical experience. Patient education or condition management experience. Bilingual in English and Spanish. Transition of care experience. Certification in Case Management, Training, Project Management, or other nationally recognized healthcare certifications. Experience managing complex or catastrophic cases.
Gathering, analyzing, and providing data for regulatory reports. Conducting assessments, care planning, and coordinating care both in member/family homes and telephonically. Representing the company to members and ensuring top-notch support. This role focuses on conducting health programs, gathering crucial data, and representing the company with professionalism and care.
