As a Chronic Care Manager, you will have the unique opportunity to provide compassionate care as either a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Enjoy flexibility in your work schedule while making a significant impact on patients’ lives.
A deep sense of compassion and commitment to patient care. Proficiency in using Electronic Medical Records (EMR). Strong time management capabilities to thrive in a remote work environment. Exceptional verbal and written communication skills. Comfort and adaptability with new technology tools. Qualifications: Licensed as an RN or LPN in Minnesota (licensing fees covered for out-of-state patients). A minimum of 4 years of nursing experience, particularly in caring for chronically ill individuals. Experience in home health or primary care settings is advantageous.
