Clinical Nurse Case Manager (RN) needed for a permanent opportunity with Yoh’s client located in the East Bay Region! **Must reside locally in the Bay Area**

Requirements

Top Skills You Should Possess: Five (5) years of nursing experience preferred, including acute care Previous Case Management experience a plus What You Need to Bring to the Table: Active and unrestricted California Registered Nurse license Graduate of an approved and accredited nursing program; Bachelors Degree in Nursing (BSN) preferred but not required Current BLS certification Five (5) years of nursing experience preferred, including acute care, but will consider a strong candidate with less experience RN Case Management and Community RN experience a plus Certification in Case Management (i.e., through CMSA) a plus Strong patient assessment skills Strong organizational skills and ability to prioritize tasks quickly and effectively Outstanding written and verbal communication skills Ability to be a strong yet tactful representative for our patients Ability to work independently and efficiently Ability to use iPhone as a professional technology tool Computer literate with strong MS Office 365 skills Valid California driver’s license, current automobile insurance, and access to a reliable vehicle Proof of personal RN liability insurance, as an addition to insurance coverage already provided by the organization (cost of approximately $100/year)

Responsibilities

What You’ll Be Doing: Responsible for all aspects of clinical care coordination and case management for client caseload, consisting of patients with a wide variety of acute and chronic healthcare needs across the lifespan The role combines working out in the field, visiting adult patients in their residence or clinical location, attending physician visits/appointments, and working remotely to manage the many aspects of care coordination New consult and admission of new patients Development of holistic and comprehensive plans of care Ongoing patient physical and psychological assessments Medication management, in-home assessments, patient necessitated nursing tasks Continued communication between the comprehensive medical/care team, patient and loved ones for continuity of care Ongoing patient, family and caregiver education Referral recommendations based on patient needs Support and coaching for office visits, including all appropriate documentation Regular review and maintenance of electronic medical records Participation in ongoing primary care conferences Schedule: 24 – 32 hours/week Patient visits will mainly be along the 680/24 corridor, as well as the Alameda/Oakland/Berkeley areas; rare/occasional visits in Livermore, Napa, SF, and the Peninsula (mileage reimbursement is included)
