Telehealth Nurse Care Coordinator (RN or LPN) - Remote Patient Monitoring We are looking for part time, full time, and PRN (as needed), RNs or LPNs to remotely monitor patients with chronic conditions. Integrative software that gathers patient data from connected devices: blood pressure cuffs, scales, blood-glucose meters, etc. 2. Clinical staff (that would be you!) that provides monitoring and care management services to patients enrolled under the physician's general supervision.
