Certifications: Current BLS certification. Experience: Minimum of 2 years of nursing experience, preferably in telehealth or remote care settings.
Responsibilities: Provide remote patient care, assessments, and health consultations using telehealth technology. Collaborate with healthcare providers to develop and implement care plans for patients. Monitor and record patient health data using video consultations and remote patient monitoring tools. Educate patients and families on healthcare plans, medication management, and preventive care via virtual platforms. Ensure compliance with telehealth protocols and maintain patient confidentiality. Participate in care coordination and case management for patients with chronic conditions or requiring follow-up care.
