The Cadence Health team seeks a Licensed Practical Nurse to support our newly established Chronic Care Management Program. The LPN will provide care, education, and support to our CCM patients to better manage their conditions. This will be our first team member supporting CCM, you will partner with our Operations and Clinical Leadership team to build out the program as we integrate it into our current RPM program. This role's schedule will be M-F 8am to 5pm Pacific Time.
