Job Description

As a Chronic Care Manager, you will maintain regular communication with patients, providing them with the necessary guidance to navigate their health challenges effectively. You will play a crucial role in bridging the gaps within traditional healthcare systems, ensuring that vulnerable individuals do not feel alone in their journey.

Requirements

Required Skills Compassionate patient care, with a focus on building meaningful relationships. RN/LPN licensure in any U.S. state (licensing fees covered for out-of-state patients). Minimum of 4 years of experience in nursing, particularly with chronically ill patients; home health or primary care experience is a plus. Proficient in using Electronic Medical Records (EMR). Strong time management capabilities, especially while working remotely. Exceptional communication skills, both written and verbal, with an emphasis on active listening. Comfort and adaptability with new technology tools.

Responsibilities

Conduct regular patient check-ins to discuss health changes and follow up on medical appointments. Build and nurture trust with patients and their families. Serve as a critical link within the patient’s care team, fostering seamless communication with healthcare providers and office staff. Advocate for patients by addressing their medical concerns and providing timely responses. Leverage technology to manage care coordination, including setting health goals and gathering essential resources. Document all patient interactions accurately utilizing Electronic Medical Records (EMR).
