Every other weekend (Saturday & Sunday)! Fully remote!! Scope: Responsible for receiving all incoming calls for home health and hospice during scheduled hours and triaging needs. Responsible for organizing and directing skilled nursing services in accordance with the standards and practice of nursing, state and federal regulations, and agency policies and procedures. Ensure time management that best utilizes agency resources while meeting client needs. Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is always maintained. Responsible for reviewing referrals and determining acceptance or rejection based on agency criteria and communicating with referral source. Requirements

Education and Experience Required: Must be licensed as a Registered Nurse in the state of Minnesota or North Dakota. Experience in home health or hospice preferred, but not required. Must complete and pass a state required background study. Able to communicate verbally and in writing to the extent required by the position. Able to physically perform the duties required by the position. Required to have the ability to read, write and speak English well enough to communicate with staff, clients and families. Responsibilities

Responsible for receiving all incoming calls for home health and hospice during scheduled hours and triaging needs. Responsible for organizing and directing skilled nursing services in accordance with the standards and practice of nursing, state and federal regulations, and agency policies and procedures. Ensure time management that best utilizes agency resources while meeting client needs. Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is always maintained. Responsible for reviewing referrals and determining acceptance or rejection based on agency criteria and communicating with referral source. Determine urgency of accepted referrals, obtain additional needed information, and schedule admission visit accordingly. Complete additional tasks as assigned to promote high quality client care and provide support to clinical teams providing care during scheduled hours. Key Responsibilities: Answer all incoming phone calls during scheduled hours and respond to patient, caregiver, and facility inquiries. Use triage protocols to assess the level of urgency and determine the best course of action for patient care. Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills. Utilize hospice standing orders to provide medication adjustments over the phone as needed. Document all triage activities and interventions in patient’s electronic medical record. Communicate with the interdisciplinary team regarding any patient related updates or changes in condition and/or plan of care. Monitor communication systems for potential weekend admissions for referrals already accepted and resumption of cares for current clients discharging from inpatient facilities. Update teams and schedule visits as indicated. Review new referrals on all platforms and determine acceptance or rejection based on agency criteria. Communicate decision with referral source and gather needed additional information. Review accepted referrals and determine need for weekend admission. Schedule visits for on-call clinical teams if indicated. Communicate all referral activity with patient access team, CTAs, and clinical managers. Assign PRN visits as needed to clinical team for weekend visits. Complete additional assigned workflow and tasks to support HCBS during scheduled hours.
