The Telephone Triage Nurse serves as a critical member of the CURIS Connect team of healthcare professionals in the provision of high quality remote clinical services and solutions. The Telephone Triage Nurse will be responsible for communicating with patients, providers, and other care team members to ensure that patients are provided with safe, timely, and appropriate assessment of needs over the phone. The Telephone Triage Nurse will provide patients with evidence-based guidance based on standardized clinical decision support tools and clinical protocols. This may include guiding patients to the most appropriate care resources or levels of care based on reported symptoms and conditions. This role requires strong clinical assessment skills, critical thinking, excellent communication skills, and the ability to work in a fast-paced environment.
Current, valid, and unrestricted nursing license (Registered Nurse). - California state license is required. • Minimum of 2 years of nursing experience, preferably in a primary care, emergency room, or urgent care setting. • Experience in telephone triage or telehealth services is preferred. • Strong clinical assessment and critical thinking skills. • Excellent verbal and written communication skills. • Proficiency in using electronic health records (EHR) and other healthcare technology. • Ability to work independently and manage time effectively in a remote environment. • PST, MST, or CST time zones are preferred.
• Conduct thorough assessments of patients' health concerns via telephone, utilizing established protocols and guidelines to evaluate symptoms, conditions, and associated concerns. • Provide appropriate clinical guidance, information, and support to patients regarding their reported health issues and concerns. • Determine urgency of patient needs and recommend appropriate next steps if emergency services are indicated. • Document all patient communication and interactions in the healthcare organization’s electronic medical record (EHR) system accurately and in a timely manner. Maintain thorough, accurate, and completed records in compliance with medical, legal, and regulatory requirements. • Handle all patient information, data, records in accordance with confidentiality policies and procedures, HIPAA information privacy and security regulatory guidelines, and compliance standards. • Collaborate with providers and healthcare team members to ensure continuity of care and effective patient centered care delivery. • Follow up with patients as necessary to assess outcomes and identify the need for any follow-up contact, appointments, etc. • Stay updated on clinical best practices, protocols, and organizational policies to provide the highest level of care. • Participate in training and development activities to enhance triage and clinical skills
